What city? There are many great boutiques in the larger cities around the US. I can recommend a few in Atlanta, but I recommend you find someone local that you can collaborate with in person. You can get the development work done remotely after it has been spec'ed out sufficiently, but you need a tight communication feedback loop with someone who you trust understands exactly what you are trying to do.
