I strongly recommend reading Reid Hoffman's LinkedIn Series B Pitch to Greylock (http://reidhoffman.org/linkedin-pitch-to-greylock/). He writes commentary on each slide surrounding his thoughts, what he would do differently, what was right, etc. Definitely worth a read for all!

As a past director @ Founder Institute & creator of a few startups I use (and recommend) this resources for each new deck I create:

It was written in 2004 but still is a solid template.
Other tips:
-shrink your deck down to a small file size and email it. Most investors will review in a down moment when Internet might not be convenient (i.e. on a plane)
-Export each slide as a jpg image then combine all of them into a pdf (this makes a VERY small file vs just saving directly to pdf from powerpoint/keynote)
-name file like this company_name-your_name-date.pdf
