What makes for a great team? Ever work on a dysfunctional team?

So many different aspects are taken into account when forming a team. I believe that what I have with my team is perfect, we function at the highest level together, just because we all excel in different areas. What do you personally think makes a great team? On the other hand, have you ever been part of a team that can't seem to find any chemistry? How was that experience, and did you guys try to fix things? Or did things just seem to fall apart. I would love to hear some experiences/stories.
A great idea is 1% of the work. Execution is the other 99%. In this course, we’ll teach you how to conduct market analysis, create an MVP and pivot (if needed), launch your business, survey customers, iterate your product/service based on feedback, and gain traction quickly.

In an great team, the team members work as clones of the 'shared team thought' especially on matters related to values, character and ethics. 

Everyone team member knows their worth and recognises others worth to the team. Also respects roles and positions .

We see more of fact based  and data driven decision making . A small learning organisation wherein each member is open to learning, unlearning and mentoring. Sometimes the team members work as roots on which the progress and achievement trees grow.

Congrats on being part of what you feel is a high performing team!  In my experience, especially as teams and organizations grow, that feeling is rare!  I've had both experiences (great team and team that sucks!) and I've been practicing what I've learned in the current teams I am a part of.  Too many learnings to write in this post but one thing I now know is that if a team operates a low performance for too long, you eventually get to a point where you just can't fix it with those players still participating.
